We can see this with the change that artists have made nowadays to smaller implants that provide more elegant curves and a more defined cleavage. A clear example of this welcome trend has been Victoria Beckham, Denise Richards or trend setter Kim Kardashian.

Implant placement has been among the top three most performed procedures since its creation in the 1960s. With approximately 1,300,000 women improving their cleavage with the placement of implants per year. Breasts are an important characteristic of femininity since the beginning of mankind. It is normal that it occupies a predominant place in feminine aesthetics and therefore in aesthetic surgery. Although breast design is performed to improve the shape of the breasts in those women who during adolescence presented a development of their breasts in a less aesthetic way (tuberous breasts), most of the times breast surgery is done to simply improve the cleavage. The cleavage is that privileged place between the two breasts of a woman; which is often highlighted by dresses or bikinis or underwear. The cleavage is defined by a region of shadow between the breasts and then enters an area of light that corresponds to the curvature of the breast and culminates at its point of maximum projection which is the nipple. The cleavage defines the breast from its surroundings which is the thorax. The cleavage reveals the attractive features of the breast with a certain mystery. Nowadays, the cleavage between the breasts is no longer the sole focus of attention, but great attention is also given to the side boob or the under boob.

These cleavages have the same characteristics of the main cleavage. In recent years the use of dresses without bra reveal the side part of the breast on the side of the arm. In this side cleavage there is also a shadow followed by a light where the breast is defined from the back through a curvature that culminates in the nipple. The same happens with bikinis or underwear that expose the lower part of the breast in which the breast is defined from the abdomen with a shadow area that is followed by a light area that corresponds to the curvature of the breast. The attractive breast is defined from the thorax, confirming its presence, standing proudly on the thorax. We can see in the following photographs a beautiful woman that the only thing that changes between the two photographs is how her left arm pushes her left breast providing more definition in her cleavage (similar function to the popular push-up bra). This increased definition of the cleavage totally changes the attractiveness of her chest by revealing a more defined relief of the breast through a relationship between light and shadow.

As you may notice we did not include among the reasons for surgery the changes incurred by age due to the effects of gravity, or the changes incurred by pregnancy, or the presence of noticeable asymmetries between the breasts (this occurs in up to 81% of women!), or the changes incurred by a larger than usual breast size. All these reasons are valid for a breast surgery, but more than a cleavage design will be needed since in these cases the breasts have suffered changes due to gravity or their position is no longer what it was in youth or their asymmetry is too important and will be enhanced with the placement of implants. For these cases, the aesthetic characteristics of the breast should be restored with a lifting and then make a design of the cleavage. We will talk about this surgery on another occasion, which is totally different.

Let's go back to cleavage design. Unfortunately many surgeons have the habit of not listening to the patient's wishes and so they choose for the patient based on their own desires and tastes: placing uncomfortably large implants. It is common to hear women say that they wanted to improve the attractiveness of their breasts and the surgeon imposed the size of the implant, which then these women feel too big and uncomfortable. On the other hand it is common to hear surgeons say instead that they make this decision because many patients regret not having larger implants than the ones that were placed. The important thing is to make sure to have a good communication to avoid surprises or misunderstandings.

Placing large implants not only cause aesthetic disadvantages, which we have mentioned before. There are also functional disadvantages such as a greater weight on the thorax and therefore back or neck pain; a shorter longevity of the result, since the heavier implants tend to fall faster (no one beats gravity); or a greater degree of complications such as skin and scarring problems.

The choice of an implant is not a matter of chance. The surgeon is responsible for making a design based on the patient's body tissues and together with the patient to choose an implant within a range of possibilities that fit her physical and aesthetic characteristics. Choosing an implant is like choosing a shoe, you have to take into account the foot.

Cleavage design surgery or breast augmentation has a quick recovery and is of great satisfaction for the patient. Complications are rare, with excessive scarring of the body around the implant causing pain and implant deformities (capsular contracture) up to 5% (5 out of 100 operated patients will present this complication). A risk of implant rupture up to 0.5% (1 out of 200 operated patients will present implant rupture) and a risk of implant associated cancer around 1 in 80,000 operated patients (much lower than the risk of breast cancer in women without implants and similar to the risk of mortality after breast reoperation). Other complications are implant malposition, sensitivity changes or implant extrusion.

It is an oupatient procedure, that is, you go home the same day, it usually takes 2 to 4 hours to perform depending on the results you wish to obtain. The procedure is performed under deep sedation controlled by an anesthesiologist.

Recovery is relatively quick, pain usually resolves within the first five days without becoming disabling. On the third day you will be able to return to your daily life with certain precautions and limitations. Healing is generally good and not very noticeable. Implants are not lifelong devices and maintenance reoperations are expected since the result obtained will change over time as it happens with the rest of the body.
